5. Providing of personal information to a third party
Unless required by the laws and regulations, or when a part of the operations is entrusted to an outside company, The Company shall not provide personal information to a third party without the consent of the individual.
6. Entrustment of personal information handling
In order to provide better services to the customer, The Company, for business management purposes, may entrust a part of its operations to an outside company. In so doing, The Company may entrust personal information to the entrusted company. In such a case, a company sufficiently satisfying the personal information protection standard shall be selected, and an agreement of commission shall be concluded therewith regarding the protection of personal information, and The Company shall thoroughly manage and supervise the entrusted company.
7. Optionality of providing personal information
It is up to individual discretion as to whether personal information is provided to The Company. However, in the event that necessary items are not provided, application to services, etc., may not be possible.
8. Items related to “procedures in response to disclosure request, etc.”
In the event that a request is made by the individual or his proxy for notification, disclosure, correction, etc., suspension of use, etc. for retained personal data use, such a request shall be handled accordingly as set forth below:

(2) Documents to be submitted (by the individual)
In the event that a request for disclosure, etc. is made, please submit the designated application according to the request item (A) and documents for personal identification (B).
i) Designated application
-Application for “retained personal data” purpose of use notification
-Application for “retained personal data” disclosure
-Application for “retained personal data” correction, etc.
-Application for “retained personal data” suspension of use, etc.
ii) Documents for personal identification
-1 copy of either a driver’s license, passport or health insurance
(3) Documents to be submitted (by proxy)
In the event that a person with parental authority, legal guardian of a minor, assistant, curator, adult guardian under the legal guardianship system, voluntary guardian under the voluntary guardianship system, or a proxy delegated by the individual, requests disclosure, etc., please submit the following documents in addition to the documents stipulated in (2).
i) For a person with parental authority or a legal guardian of a minor
-1 copy of the family register (a copy of an insurance card which indicates the dependent family Members possible)
ii) For an assistant, curator, adult guardian or a voluntary guardian
-1 copy of the certificate of registered matters regarding guardianship (to be acquired at the Legal Affairs Bureau)
iii) For a delegated proxy
-1 copy of the power of attorney (with a registered seal)
-1 copy of the seal registration certificate of the principal
(4) Handling fees, etc.
The handling fee for the request for disclosure, etc., shall be 700 yen per application by 1 individual (1 data subject). Please enclose 700 yen equivalent in postage stamps with the application
* Notification shall be sent in the event that there is a deficiency in the handling fee; however, if the payment is not made within the prescribed period, the request for disclosure, etc., shall be nullified.
(5) Method of reply
The request shall be sent in writing to the address of the applicant indicated on the application.
(6) Purpose of use
Individual information acquired through request for disclosure, etc., shall be handled within the necessary scope in addressing the request for disclosure etc. After the completion of response to the request for disclosure, etc., the submitted document shall be stored for 2 years, then destroyed.
* When the request for disclosure, etc. is denied
Request for disclosure, etc. cannot be complied with in the respective cases set forth below. In the event that it has been determined that information cannot be disclosed, the reason shall be indicated and notified. Please note that even in the event of a non-disclosure, etc., the designated handling fee shall be charged.
- In the event that personal identification cannot be confirmed, such as when the address indicated on the application, the address indicated on the submitted documents for personal identification, and the address registered with The Company do not coincide, etc.
- In the application by a proxy, in the event that the authority of proxy cannot be confirmed.
- In the event that there are deficiencies in the designated application documents, and the corrective measures for the deficiencies are not taken despite being requested to do so.
- In the event that the information requested for disclosure, etc. does not correspond to “retained personal data” (information etc., which is to be eliminated within 6 months from acquisition)
- In the event that the providing of information is likely to endanger the life, body, property, or other rights and interests of the individual or a third party
- In the event that the providing of information is likely to cause significant impedance to the appropriate implementation of The Company’s operations
- In the event that the providing of information will violate other laws and regulations
